CDL/DOT Physicals

The Department of Transportation (DOT) requires health exams for commercial motor vehicle (CMV) operators driving vehicles that weigh five tons or more. Dr. Panza is a Certified Medical Examiner through the Federal Motor Carrier Safety Administration (FMCSA).

  • Please bring your driver’s license and a copy of current medications. If possible please text your drivers’ license to 360.824.3404 prior appointment and confirm if address on ID is current.

  • Please complete your DOT forms prior to your visit; Click here; DOT FORMS (Print or Email to clinic).

  • Fee: $175. We do have billing accounts with some local employers. These cannot be billed to insurance.

  • Other documentation you may need

    • See the FMSCA form page for pertinent forms you may need.

    • Sleep apnea: Please bring CPAP usage report to your appointment if you have sleep apnea

    • Diabetes: A1c lab work within last 3 months. If insulin treated - Insulin FMSCA Forms

    • Vision impairments: If you have a disqualifying vision condition your ophthalmologist will need to fill out a Vision Waiver for your CDL exam

    • Controlled Prescriptions: If you are taking a controlled substance (Adderall, Xanax, Opiates) you will need your prescribing provider to clear you with a FMSCA Medication Waiver

Saturday CDL/DOT Physicals

Starting January 2024 we will be offering once monthly Saturday services for CDL physicals.

Please request this specifically when scheduling as this will not be available on our online scheduling platform nor advertised. We are attempting to accommodate drivers with scheduling difficulties.
